radicle: Reject COB changes with forged identity
Each COB change names an identity commit in its `Rad-Resource`
trailer. Patches and issues check the rights of the author against the
document in that commit. Nothing checked that the commit is a revision
of the repository identity. Any user could write a commit with a
document that names them as a delegate, cite it, and act as a delegate.
- Add `ReadRepository::is_identity_revision`, which loads the identity
COB and accepts only its accepted revisions
- Reject the change in `Op::identity_doc` if its resource is not an
accepted revision
Honest clients cite the identity head, which is an accepted revision,
so honest COBs evaluate the same.
Note: A removed delegate can still cite an older revision that names
them. This fix does not stop that.

radicle: Test forged identity resource on COBs
- Add test helpers that forge an identity commit and store a raw
change that cites a given `Rad-Resource`
- Let a non-delegate assign on a patch and label an issue, first with
the real identity head, then with a forged commit that names them a
delegate
- Expect both changes to be rejected
- Currently fails: the forged commit gives the non-delegate delegate
rights
